38-year-old Tommy Johnson is a native from Louisiana, and when he first appears in the episode, he weighs a massive 641 pounds. He is in love with his fiancee Amanda and is worried that if he does not get back on track, she might leave him. This is what he says during an emotional moment in the show: “A couple of years ago, I proposed to her and I committed to her to lose the weight, so I can give her a proper wedding. She wants to be able to walk down the aisle … but that was years ago I made that commitment to her and I still haven’t fulfilled it. I feel like a burden, I don’t feel like a man. I haven’t even ever taken her on a real date.”

However, Amanda has her own backstory of being involved with morbidly obese men and her ex even died due to his heavy weight. She says she does not want to go through that trauma again. It even appears that Amanda is kind of a food enabler for Tommy but the truth is, Tommy has battled with obesity since his childhood.

After Tommy meets Dr. Now, he starts working amazingly hard, manages to lose 70 lbs, and is approved for the sleeve surgery. However, to remain in the program and lose further mass, he is prescribed a therapist. He is finally able to shed 218 pounds toward the end of the episode.
